To Obtain Services

Referrals are accepted from a variety of sources including physicians, osteopaths, dentists, podiatrists, case managers, vocational counselors, and attorneys. Some services require a written prescription before treatment can be initiated. If you are uncertain about whether you will benefit from our programs, our staff will evaluate your needs and help you secure the appropriate medical prescription.

Payment for Services

Most insurance companies provide reimbursement for rehabilitation services. Prior to beginning treatment, our staff will verify coverage. We will file your insurance, however, you are expected to pay your unpaid deductible and appropriate co-payment at the time of service. We accept private insurance (including PHP Products), Medicare assignment, Tenncare and worker's compensation. Our staff will be happy to discuss payment plans to meet your individual needs.

Office Hours

Appointments for clinical treatment can be made Monday through Friday from 8:00 a.m. to 5:00 p.m. by contacting the office nearest you. If these time frames are not conducive to your schedule, we will try to work out a flexible schedule with you.

Hours of operation for the Jump Start Community Program are 6:00 a.m. to 8:00 p.m. Monday through Friday and 8:00 a.m. to 4:00 p.m. on Saturday.
